HIV Prevention • PrEP Methods

Daily PrEP vs
on-demand PrEP.

PrEP is not one protocol. There are two clinically validated dosing methods, and the right one depends entirely on who you are, how you’re exposed to HIV risk, and how predictably you can manage a dosing schedule. Choosing wrong isn’t just inconvenient, it can mean taking the less appropriate method for your biology.

Side by Side

The two methods compared.

Daily PrEP
  • One tablet, every day
  • Continuous protection once levels are established
  • Full effect: 7 days (anal sex), 21 days (vaginal/injection)
  • Suitable for all genders and all exposure types
  • No planning required around sex
  • WHO- and Nigeria-guideline recommended for most people
On-Demand (2-1-1)
  • 2 tablets 2–24 hrs before sex, then 1 tablet at 24 hrs, 1 at 48 hrs
  • Protection only around planned sexual events
  • Fewer total pills; lower cost over time
  • Only appropriate for MSM, not women or injection drug users
  • Requires ability to plan and follow the schedule precisely
  • Not appropriate for frequent or unpredictable sex

The On-Demand Schedule in Detail

How 2-1-1 works, step by step.

1Take 2 tablets between 2 and 24 hours before sexual activity.
2Take 1 tablet exactly 24 hours after the first dose.
3Take 1 tablet exactly 24 hours after the second dose (48 hours total after dose 1).
4If sex continues across multiple days, take one tablet each day until two days after the last sexual exposure, then stop.
Missing the pre-sex double dose means no protection. Unlike daily PrEP, where one missed dose rarely matters much, skipping or delaying the 2-1-1 pre-dose means drug levels never reach the tissue concentration needed for protection. The schedule is non-negotiable for on-demand to work.

Why On-Demand Is Not for Everyone

On-demand doesn’t work for vaginal sex. Here’s why.

The short window of the 2-1-1 schedule simply isn’t enough time for PrEP to build up protection in the tissues involved in vaginal sex. Those tissues need 21 days of daily dosing to reach protective levels, that’s why daily PrEP has a longer lead-in time for women than for anal sex. A pre-event double dose can’t shortcut that. The evidence for on-demand PrEP comes entirely from trials in men who have sex with men, and it has never been validated for women, transgender women, or anyone whose primary exposure route is vaginal sex. This isn’t a grey area. It’s the wrong tool for that context.

For MSM who have infrequent, predictable sex and can follow the schedule reliably, on-demand is a legitimate, evidence-based choice. For everyone else, daily PrEP is the right method.

Frequently Asked Questions

Common questions about PrEP dosing methods.

QWhat is the difference between daily PrEP and on-demand PrEP?

Daily PrEP involves one tablet every day, providing continuous protection. On-demand (2-1-1) PrEP involves a double dose before sex and single doses at 24 and 48 hours after. On-demand is only appropriate for MSM and is not recommended for women, transgender people, or people who inject drugs.

QCan women use on-demand PrEP?

No. On-demand PrEP is not appropriate for women or anyone whose primary HIV exposure route is vaginal sex. The short pre-sex dosing window doesn’t build up enough protection in the tissues involved in vaginal sex. Women need daily dosing for 21 days to reach full protection, that cannot be achieved with the 2-1-1 schedule.

QWho is on-demand PrEP best for?

On-demand PrEP is appropriate specifically for MSM who have infrequent, predictable sexual activity and can reliably follow the 2-1-1 dosing schedule. It is not appropriate when sexual activity is frequent, unpredictable, or involves vaginal exposure.

The best PrEP method is the one suited to your biology and your life.

For most people in Nigeria, that’s daily PrEP. For some MSM, on-demand is a legitimate option. Our team will help you identify which.
